Preparing for Your First Visit at Southpaw Animal Clinic of Madison


Before the Appointment

  • Complete New Client Form
  • Forms not completed 24 hours prior to the appointment may be cancelled.
  • Provide Medical Records
  • Vaccination history
  • Previous diagnoses, surgeries, or conditions
  • Labwork or imaging results
  • Medication list (including any supplements)
  • Bring a Valid Photo ID
  • Required for controlled substance prescriptions and client verification.
  • Complete Pre-Visit Questionnaire
  • Check your email or text reminder


What to Bring

  • Your pet on a Leash or in a Carrier
  • Dogs on flexi-leashes will be given a slip lead upon arrival for safety.
  • Fecal Sample
  • Bring to any scheduled appointment
  • List of Questions or Concerns

On Arrival

  • Check In at Front Desk
  • Let us know if your pet is anxious or prefers to wait in the car--we are happy to accommodate.
  • Meet Your Veterinary Team
  • A Licensed Veterinary Technician (LVT) or veterinary assistant will begin with vitals and a history before the doctor joins you.


Helpful Tips to Make the Visit Smoother

  • Bring Favorite Treats or Toys
  • This helps make the visit more comfortable for your pet.
  • Bring a Stool and Urine Sample
  • If you'd like to walk your pet before the appointment, ask the front desk if a urine sample will be needed. A veterinary care team member can assist in urine sample collection.
  • Limit Food Before the Visit
  • We use treats for comfort and cooperation
  • Practice Handling at Home
  • Gently touching your pet's paws, ears, or mouth in the days before the visit can help prepare them for the exam.
