Pre-Visit Questionnaire
Southpaw Animal Clinic of Madison
Preparing Your Pet for a Visit to Southpaw Animal Clinic of Madison
At Southpaw Animal Clinic of Madison, we believe in providing comprehensive care for your beloved pets. One way we do this is through our pre-visit questionnaire. This tool helps us understand your pet's needs better, ensuring a smooth and efficient visit. Don't hesitate to contact us today if you need any assistance with this process.
📝 Why We Require a Pre-Visit Questionnaire for Every Pet
To ensure your pet receives the highest quality care and a stress-free experience, we ask that you fill out this Pre-Visit Questionnaire before each appointment. This brief form helps our team better understand your pet’s current health, behavior, and needs.
🐾 Why It's Important — Every Time
Even if your pet has been seen before, their health, behavior, and routine can change between visits. Here’s why completing the form each time is essential:
- Behavioral insights: Helps us prepare for any anxiety, fear, or aggression to keep your pet calm and comfortable.
- Updated health info: Alerts us to recent changes in appetite, energy levels, symptoms, or medications.
- Customized care: Allows us to tailor the visit to your pet’s specific needs that day.
- Safety for all: Ensures a smoother visit for your pet, our staff, and other patients in the clinic.
🐕 One Pet = One Form
Each pet is unique! If you’re bringing multiple pets, please complete a separate questionnaire for each one. This allows us to review their individual histories and needs thoroughly.
💻 How to Complete It

🧳 What to Bring to Every Appointment
Whether it’s a routine check-up or something more urgent, here are a few things to bring with you to help us give your pet the
best care possible:
💩 Fresh Stool Sample
Please bring a stool (fecal) sample from the last 24 hours. This helps us check for intestinal parasites, which are common even in pets who appear healthy.
Pro tip: Use a clean plastic bag or container and refrigerate if you collect it ahead of time.
🍖 Tasty Treats
Bring your pet’s favorite snacks! Treats help make the visit a more positive and less stressful experience, especially during exams, vaccines, or blood draws. If your pet has dietary restrictions, feel free to bring approved options.
🍽️ Come a Little Hungry
Try to avoid feeding your pet a full meal right before the appointment. A slightly hungry pet is usually more food-motivated and responsive to treats during their visit.
📋 Any Medications or Supplements
Bring a list or photo of any medications, supplements, or preventatives your pet is currently taking, including dosages and how often they’re given.
📁 Previous Records (if not already sent)
If you're visiting us for the first time or have been to another clinic recently, bring any recent records or test results, or have them sent to us ahead of time.
